The actual user will need to sign the liability waiver which will be provided with the items you pick up and returned when you bring back the items you have borrowed.  Please use the plastic container which is mounted by the door of the north Care Closet barn to return the signed waiver. 

I, the USER of this equipment, hereby accept from VASHON CARE NETWORK (VCN) the used medical equipment that I have been given. I agree that this is an on-going waiver and extends to equipment that I receive today and may receive in the future. I ACCEPT RESPONSIBILITY to seek advice from my personal medical advisors, and to use the equipment in accordance with their instructions.

NO WARRANTY. VASHON CARE NETWORK makes no warranty or representation concerning this equipment. VCN excludes all warranties whether express or implied which might conceivably apply to this equipment,including the implied warranties of merchantability and fitness for a particular purpose. This equipment is being provided with all faults whether apparent or latent.

NO LIABILITY. This equipment is given “as is” and without a warranty of any kind. In no event shall VCN be liable for general, incidental, or consequential damages resulting from use of this equipment. VCN does not know how the equipment may have been used in the past and it may contain defects or problems of which VCN is unaware. VASHON CARE NETWORK includes the corporation, its employees, contractors, volunteers, officers and directors.

Note: The user is responsible for proper operation and maintenance of the equipment, such as new batteries and safe storage. You can use your equipment for two weeks without prior arrangement for more time. We ask that you return it to us promptly when you are finished using it – continuing the cycle of donations and helping others. VCN’s inventory is not for sale or storage for future use. Our mission is to help as many people as we can through multiple uses of our inventory.

Entiendo que esta es una exención continua y se extiende al equipo que recibí hoy y que puedo recibir en el futuro. ACEPTO LA RESPONSABILIDAD de buscar asesoría del personal médico para utilizar el equipo de acuerdo con sus instrucciones.

 

SIN GARANTÍA. VASHON CARE NETWORK no ofrece garantía ni representación con respecto a este quipo. VCN excluye todas las garantías, ya sean expresas o implícitas, que posiblemente puedan aplicarse a este equipo, incluidas las garantías implícitas de comerciabilidad e idoneidad para un propósito particular. este equipo ha sido entregado con defectos, ya sean aparentes o latentes.

 

NINGUNA RESPONSABILIDAD. Este equipo se entrega “tal cual” y sin garantía de ningún tipo. En ningún caso VCN será responsable por daños generales, incidentales o consecuentes que resulten del uso de este equipo. VCN no tiene manera de saber cómo este equipo pudo ser utilizado en el pasado y puede contener defectos o problemas de los cuales VCN no tiene conocimiento. Al referirnos a VASHON CARE NETWORK, incluimos la corporación, sus empleados, contratistas, voluntarios, funcionarios y directores.

 

Nota: El usuario es responsable del correcto funcionamiento y mantenimiento del equipo, como baterías nuevas y almacenamiento seguro. Puede utilizar su equipo durante dos semanas sin previo acuerdo para más tiempo. Le pedimos que nos lo devuelva de inmediato cuando termine de usarlo, continuando el ciclo de donaciones y ayudando otros. El inventario de VCN no está a la venta ni se almacena para uso futuro. Nuestra misión es ayudar a muchas personas a través de múltiples usos de nuestro inventario.
